## Document formats and sizes

Every record is listed with the format and file size given in the original index. Those figures are metadata carried over from the index itself; no document file is stored on or served from this site.

## Retired data-access routes

The original site also carried three query-service sign-in pages and their dispatcher endpoint. Those services no longer exist and are not reproduced here. Their addresses now lead to this page so that old links continue to resolve.

## Non-affiliation

This site is an independent archive index. It is not a government service and is not affiliated with, endorsed by or operated by any state health department, public-health agency or the Behavioral Risk Factor Surveillance System programme. No figure on this site is presented as official statistics, and no health, screening or treatment guidance is offered.

## Accessibility

This site uses one heading order per page, real table headers on every record list, underlined links, a visible focus outline, and no motion or script-driven behaviour. If a record is unreadable in your browser, the same information is present in the page source as plain text.
